When an AI system hallucinates a quote, the underlying retrieval is usually fine — the source document still exists and the right context can be pulled. The hallucination lives in how the result is rendered to the user, not in what was looked up. This decoupling matters: you can fix the presentation layer without rebuilding the retrieval pipeline.
